• Eczema (Atopic Dermatitis): Flare-ups of mild-to-moderate, aged 2–65 years.

• Impetigo (School Sores): For patients 2 years and older.

• Shingles (Herpes Zoster): For patients 18 years and older.

• Psoriasis (Mild Plaque): Flare-ups of mild plaque, for patients 18 years and older.

Conditions we can treat:

• Cost: $25 plus the cost of medication

• Assessment required before treatment is supplied

• Availability: Call ahead to check participating pharmacy locations and appointment times
